Reimbursing mHealth Investments For Chronic Care Management
When it comes to managing multiple chronic diseases, care coordination can be costly and difficult to manage without the use of technology. Recent updates to CPT Code 99490 is making it easier for providers to be reimbursed for chronic care management that’s facilitated using digital technologies:
Now all an eligible practitioner or physician needs to get is: the patient’s agreement, to create a patient-centered care plan, provide the patient with a copy of the plan and make sure that the plan is electronically available to anyone within the practice providing the Chronic Care Management (CCM) service and outside the practice as appropriate. The care team will then qualify for the new reimbursement for up to 20 minutes per calendar
month of non-face-to-face care. The key is the communication between the patient and any caregiver. The care can be delivered via secure messaging, secure internet, or other asynchronous non-face-to-face consultation methods, but not by fax.
